About the Event

Join the CIRO North West & Wales Area Council in-person or online for this engaging talk by

Guest Speaker Colin Haynes

What if “human error” is not the end of an investigation, but the beginning of a better one? Starting with a smoke-filled rail incident at Oxenholme, this talk traces a ten-year journey as Colin Haynes brought a passion for Aviation Safety to a major global player in the rail industry.  A chance career change from Information Systems and Technology to Quality and Safety led him to champion the shift from fault-finding to a fairer, more effective approach to safety, quality and organisational learning.

Through case studies and practical methods—including human factors, structured problem solving, systems thinking and Just Culture, Colin discusses how everyday pressures, weak signals and accelerating change can quietly erode safety margins. Attendees will leave with practical questions and accessible tools to investigate beyond the visible mistake, encourage people to speak up and turn uncomfortable events into lasting improvements. A thought-provoking session for leaders, investigators and anyone interested in creating safer, more resilient organisations.
